Overview
ARA-290 (Cibinetide) is a synthetic 11-amino acid peptide derived from the three-dimensional structure of erythropoietin (EPO). It selectively activates the innate repair receptor (IRR), a heterocomplex of EPO receptor and beta common receptor (CD131), without stimulating erythropoiesis (red blood cell production). This separates the tissue-protective effects of EPO from its blood-thickening effects.
Mechanism of Action
ARA-290 binds to the innate repair receptor, triggering anti-inflammatory and tissue-protective signaling cascades. This includes suppression of pro-inflammatory cytokine production, promotion of anti-inflammatory macrophage polarization (M1 to M2 shift), and support of nerve fiber regeneration. Unlike EPO, it does not activate the classical EPO receptor homodimer, so it does not stimulate erythropoiesis.

Clinical studies have explored ARA-290 in small fiber neuropathy, sarcoidosis, type 2 diabetes with neuropathic pain, and chronic inflammatory conditions. Its ability to promote nerve fiber regeneration is of particular interest.
